Scholarship Request Form
In order for the Peoria Public Schools Foundation to send your scholarship funds to the undergraduate institution of your choice you MUST complete all parts of the form and attach an official enrollment verification letter from your Registrar’s Office. Once received, the Foundation will process payment and send to your institution’s Financial Aid Office where it will be applied toward your tuition, books or fees. Please allow 2 weeks for processing after your request has been received.
