Information for Recipients
Acceptance Agreement Form
Click on the button below if you need a copy of the Grant Acceptance Agreement form. A signed copy of this form must be sent to matthew.violet@psd150.org in order to start using the grant funds.
Reimbursements
In order to receive reimbursement for any purchases you have made for your project, please complete the Board of Education Direct Payment Request, include all receipts, and submit to PPS Grant Specialist, Joe Gualandri, at joseph.gualandri@psd150.org. All reimbursement requests must be completed by May 31, 2025.
Photographs
We strongly encourage you to take photographs of your project in action. Please attach these pictures to your grant evaluation or email to matthew.violet@psd150.org. Make sure to sign a photo release form and e-mail it to Matt before you upload or send any pictures of students.
Evaluation
An online evaluation is required at the conclusion of your project and must be submitted no later than June 8, 2025. If you do not submit an evaluation, you will not be eligible for future grant opportunities from the PPS Foundation
